The bellow form can be used to initiate requesting: any Personal Invitations, Organizational Invitations, Sanctuary Services (by "Leaders"), Personal One-On-One or Family Visits, Sanctuary Baptisms, Outdoor Baptisms (i.e) pools, lakes, YMCA, Organized neighborhood Baptisms or just a Saturday / Sunday Service Experience..., etc. The document has been made available for viewing BEFORE you download to assure that you are downloading the correct document needed (for your desired purpose).
