Below is a TRUE SAMPLE of the documentation that we here at Female Scribe of God, LLC will use to collect information from you and it can also be use by you to submit to let us know who you are, what date you would like The Ambassador to be present and what is it you would like her to participate in, (i.e): to Speak, Pray, Baptize, participate in Communion, etc.etc. The document has been made available for viewing BEFORE you download to assure that you are downloading the correct document needed (for your desired purpose). The original document will have a unique WATERMARK to prevent tampering.
