BREAKING BREAD RELEASE OF LIABILITY WAIVER FOR MINORS (UNDER 16)
Below is a TRUE SAMPLE of the documentation needed to have a child under the age of sixteen (16) to participate in The Breaking of Bread / Communion in the presence of The Madam Ambassador / Tina Mary Martin (after) being invited by the adult who is "responsible for the underaged child.
